OPT Student Information
  • It is required that your employment records remain up to date in SEVIS. Please submit this request each time your information changes.
STEM OPT Extension Application
  • F-1 students who completed a bachelor's, master's, or doctoral degree in select government-designated science, technology, engineering, or mathematics (STEM) fields and are currently working with post-completion OPT may be eligible to apply for the STEM OPT extension of 24 months of additional employment if they have a job or job offer from an E-Verify employer and meet all the required criteria. Please refer to the USCIS STEM OPT Extension site to see if you meet the STEM extension criteria.
Travel Form

SEVIS Transfer Out
  • This request is used to notify OISS of your intent to transfer your F-1 immigration status to another U.S. institution. We recommend you meet with OISS to submit this request. Please complete and submit this request to OISS along with proof of your admission to the other institution, in order for the OISS to release your SEVIS record to your new school.
Letter/Form Request
  • The above form will inform OISS of a student's need for specific requests. 
New/Updated Form I-20 Request
  • The above form will help OISS determine if an individual is eligible for a new Form I-20 (example: change of level).
  • Note: this is not for updated Form I-20's due to employer updates. If you are a student who is on OPT with an employment update for OISS, please visit the OPT Students section on your home page in order to inform us of your employment updates.
